Serious Illness Policy Application (download pdf)

The WorldCare Global Health Plan offers two serious illness policies that provide medical coverage for U.S treatment of certain serious conditions facilitated by WorldCare’s e-Consultation Services (Telemedicine Services) and rendered by WorldCare ConsortiumSM hospitals. It provides the options of Premier and Premier Plus coverage for serious illness, with optional riders for organ transplants, travel and care coordination.

Our product has been designed to give you peace of mind by providing you this unsurpassed medical coverage for necessary medical services without hidden costs or loopholes.

PREMIER POLICY
The Premier Policy covers pre-approved, medically necessary and appropriate medical/ surgical treatment and consulting procedures for serious health conditions including cancer, cardiovascular, neurovascular and major orthopedic (joint replacement) disease when provided at a U.S. WorldCare GHP Network hospital. WorldCare pays directly to the Network hospitals and providers for pre-approved, medically necessary and appropriate treatment of an insured person for the medical/surgical & consulting procedures specified below.

PREMIER PLUS POLICY
The Premier Plus Policy includes Premier coverage and adds limited Chemotherapy and Radiotherapy coverage.

POLICY BENEFITS

Medical Benefits

  • Medical/surgical & consulting services related to the following procedures:
  • Heart & Circulatory System Surgery
  • Coronary artery by-pass surgery, heart valve surgery and correction of congenital heart anomalies in covered newborns
  • Angioplasty and major vascular surgery
  • Cancer
  • Surgery to remove all or part of a malignant tumor
  • Chemotherapy and radiation therapy (Premier Plus)
  • e-Consultation for non-surgical cancer treatment
  • Major Joint Replacement
  • Includes replacement of natural hip, knee and ankle joints
  • Neurosurgery
  • Removal of malignant tumor
  • Removal of non-cancerous, life-threatening tumor
  • Repair of intracranial blood vessel abnormality (aneurysms)

WorldCare e-Consultation

  • Policies include access to WorldCare’s e-Health facility, which provides for secure electronic transmission of diagnostic quality imaging and other medical information, including courier delivery of specimens to WorldCare Global Health Plan Network hospitals for specialist review. WorldCare e-Consultations are designed to assist attending physicians with confirmation of a diagnosis and the development of an optimal treatment plan.

Non-Medical Benefits

Optional Travel and Living Allowance
WorldCare provides a Travel and Living Allowance rider which provides round trip business class air transportation for the insured and a companion, and a per diem living allowance during covered treatment because we know how important the support of a companion can be during a difficult time.

WorldCare will arrange business class air transportation for the insured and one companion, and provide a limited per diem living allowance. Each option requires an additional premium and can be added to either the Premier or Premier Plus policy:

  • Option A: Provides a living allowance of $200 per day for up to 30 days
  • Option B: Provides an allowance of $350 per day for up to 45 days

WorldCare offers a fixed indemnity benefit to its insured persons under these conditions:

  • Insured chooses to have covered, pre approval treatment performed in his home country
  • Patient is unable to travel to a Consortium hospital for health reasons

The following of Fixed Indemnity Benefits apply:

These cash benefits are limited to one indemnity payment per covered health condition per insured for the life of the policy, and are in lieu of all other policy benefits for that condition.

Care Coordination Services
WorldCare understands that a serious illness and hospitalization can be a difficult experience. That is why we offer Care Coordination, a program of support services designed to help you through an episode of care from initial e-Consultation through admission, discharge and follow up.

Care Coordination is a comprehensive program of integrated support services designed to facilitate the insured’s entire episode of care. Services include:

  • Escort from U.S. airport to hospital or other accommodations
  • Assistance with hospital admissions and physicians’ appointment scheduling
  • Escort to U.S. airport after discharge
  • WorldCare Regional Medical Director’s assistance in relaying post-discharge treatment plan and in coordinating follow-up care with local attending physician.

Policy Limitations and Exclusions
Treatment must be pre-approved, medically necessary, appropriate and performed at a WorldCare Consortium Hospital:

  • No deductibles for covered benefits
  • No dollar limit for an individual episode of care (subject to lifetime maximum)
  • 45 day limit for episode of care
  • Eligible issue ages 0-59 and are renewable to age 75
  • Lifetime maximum benefit $1,000,000
  • Lifetime maximum benefit $100,000 for Chemo/Radiation (Premier Plus)
  • Covered exclusions include claims resulting from and or related to:
    • War or war-like activities
    • Man-made or natural disasters
    • Claims attributable to the commission of a crime or self-inflicted wounds
    • Cosmetic surgery
    • Sexually transmitted diseases including AIDS/HIV
    • Experimental treatments, including organ transplants, bone marrow transplants and chemotherapy (except where coverage is provided under Premier Plus policy)
    • Non-cancerous spinal conditions
    • Conditions that pre-existed the coverage effective date
    • Covered benefits commence 180 days after policy effective date

The Policy is issued based upon WorldCare’s evaluation of the Serious Illness Policy Application (download pdf), Medical Questionnaire, and medical Screening tests, which may be required of applicant and/or each dependent applicant for whom coverage is desired, and, payment of premium in full.

Policy is an annual contract and annually renewable. Coverage will not be canceled or premium rates increased due to claims on an individual basis. Coverage may be canceled or premium rates changed if the same action is taken on all insured persons of the same class in the insured country of residence. Premium rates are determined based on age class for each covered person.
